Parents, if you are having trouble completing the Medical Verification Form on line, remember, you must first create a Parent Account, and then click on each of your students under your account. Click on Instructions below for Creating a Parent Account.
If you are still unable to create a Parent Account, please Print the Medical Verification Form from the file below and return it to your child's school nurse.
