Diabetes is a chronic disease in which the body does not make or properly use insulin. Insulin is a hormone needed to convert sugar, starches, and other food into energy. People with diabetes have increased blood glucose (sugar) levels because they lack insulin, have insufficient insulin, or are resistant to insulin's effects.
Parents of children with diabetes please have your child's diabetes care provider complete the forms below and return them to the school nurse prior to the start of the school year.
In addition to the forms below information about BAQSIMI, a nasal form of glucagon, is attached for your information. Some students may be carrying this to be used in case of life threatening low blood sugar (hypoglycemia).
