The Pennsylvania School Health Act mandates that all children upon original entry into school (kindergarten or 1st grade), 3rd grade, and in 7th grade shall have a dental examination. It is preferred that this exam be completed upon entry into the current grade but it may be completed and returned up until May 1st of this school year.
Please have this report signed by your family dentist and return it to your School Nurse.
